New forms are now available on the DCOZ website!
For your convenience, the DC Office of Zoning (DCOZ) has uploaded its official forms for filing or participating in BZA and ZC cases. New forms are coded in blue while revised forms are in red. Below please find a list of all forms:

To access these forms from the DCOZ website, click on Application Forms under "Information" in the left side navigational bar. Select the needed BZA or ZC form from the pull-down list and click "OPEN". At this time, you may fill out the application online. However, the form cannot be submitted electronically; you must print and mail or deliver the application to the Office of Zoning.
