Map Amendment Form 101

Instruction for Form 101 - Application/Petition to Amend the Zoning Map:

Pre-Application Requirements: 

At least forty-five (45) days prior to filing a map amendment application, the applicant must serve a written notice of intent (NOI) to file the application on the affected ANC and on the owners of all property within two hundred feet (200 ft.) of the perimeter of the property in question. 

In a map amendment application, the applicant must make all reasonable efforts to attend a duly noticed meeting of the affected ANC during the forty-five (45) day notice period. 

Application/Petition Requirements: 

Each application/petition for approval of a map amendment must meet the below requirements before it will be accepted by the Office of Zoning for processing: 
 

  1. All applications/petitions shall be made pursuant to this form. If additional space is necessary, use separate sheets of 8½" x 11" paper to complete the form (drawings and plans may be no larger than 11" x 17").
  2. Form 101 must be filed electronically using the Interactive Zoning Information System (IZIS).
  3. A map amendment application/petition will not be processed until the application/petition is complete and all required fees are paid in accordance with the applicable fee schedule prescribed in Subtitle Z, Chapter 16. (Check or money order is payable to the “DC Treasurer” or credit/debit card payments are acceptable.)
  4. Below is a list of all the documents that must be submitted in PDF format at the time you file your application/petition. Please have these documents ready for uploading before you start your application.
    1. A completed application/petition form;
    2. A certified surveyor’s plat of the subject property prepared by the Office of the Surveyor;
    3. A map showing the location of the properties including, the existing zoning, the zoning of adjacent properties, and all proposed changes of zoning;
    4. A detailed description of the map amendment’s consistency with the Comprehensive Plan and any other adopted public policies and active programs related to the subject site;
    5. If a larger collection of properties is the subject of this application/petition, making the collection of all owners’ signatures impracticable, please explain on a separate sheet of 8½" x 11" paper the reason that obtaining all signatures could not be accomplished.
    6. For a map amendment application, a statement certifying to whom and in what manner the required NOI was given. The applicant shall also indicate what meetings or discussions were held with the community and the OP and the affected ANC as well as any changes that resulted from community input; and
    7. For a map amendment application, the names and mailing addresses of the owners of all property within 200 feet, in all directions from all boundaries of the property involved in the application, and two (2) sets of self-stick labels of the names and mailing addresses of the owners of the properties. (Note: This information is most readily available from the D.C. Department of Tax and Revenue, Tax Assessors by using the following link:  https://mytax.dc.gov/_/ .  Select “View More Options” under the Real Property section, then “Request a Radius Map” under the Real Property Quick Links section).
  5. Please note, after you submit this electronic filing, you will be required to submit two paper copies of the application with supporting documents, unless you are including plans, in which case eight copies are required. Also, at the time of filing you will be required to serve a copy of your application on the Office of Planning and the affected Advisory Neighborhood Commission 

    The application will not be accepted for processing unless accompanied by a certificate of service demonstrating that a copy of the application and all accompanying documents have been served upon: 


    Note: All applications are referred for review and recommendation to the Office of Planning (OP) and the Advisory Neighborhood Commission (ANC) within which the affected property is located. Their reports are given “great weight” in the Zoning Commission decision-making process. Applicants are strongly encouraged to contact these agencies to discuss the merits of their application/petition. OP can be reached at (202) 442-7600. ANC information can be ascertained by contacting the Office of Advisory Neighborhood Commissions at (202) 727-9945.
    1. The Office of Planning; and
    2. The affected ANC.
