Instructions for Form 108 - Application for Approval of Campus Plan/Further Processing or Medical Campus Plan

Pre-Application Requirements: 

At least forty-five (45) days prior to filing a campus plan/further processing or medical plan application, the applicant must serve a written notice of intent (NOI) to file the application on the affected ANC and on the owners of all property within two hundred feet (200 ft.) of the perimeter of the property in question. 

In a campus plan/further processing or medical plan, the applicant must make all reasonable efforts to attend a duly noticed meeting of the affected ANC during the forty-five (45) day notice period. 

Application Requirements: 

Each application for approval of campus plan/further processing or medical plan must meet the below requirements before it will be accepted by the Office of Zoning for processing: 

  1. All applications shall be made pursuant to this form. If additional space is necessary, use separate sheets of 8½" x 11" paper to complete the form (drawings and plans may be no larger than 11" x 17").
  2. Form 108 must be filed electronically using the Interactive Zoning Information System (IZIS).
  3. At the time of filing this application, the Applicant shall pay a filing fee in accordance with the Subtitle Z, Chapter 16. (Check or money order is payable to the “DC Treasurer” or credit/debit card payments are acceptable.)
    • A completed application form;
    • A certified surveyor’s plat of the subject property prepared by the Office of the Surveyor;
    • Facilities Plan:
      • Existing conditions, including existing topography, the location of any wetlands, and the location and size of existing trees to be removed on or adjacent to the property;
        • Building mass and height;
        • FAR for current campus; and
        • Current building uses;
    • Proposed building uses.
      • Future needs/capital improvements (10 yrs.);
      • Building mass, including FAR, height, and bulk of all new or proposed buildings;
      • FAR for entire campus with proposed building(s); and
      • Proposed building uses.
    • Student count for every student on campus, including full-time, part-time, foreign, certificate/non-degree, single course, night programs, and executive program students (if applicable);
    • Employee count for every employee, including full-time, part-time, and administration;
    • Neighborhood Context:
      • Edge conditions/Border transitions;
      • Community relations;
      • Identification/Mitigation of impacts;
      • Noise, amplified sound, lighting, etc.; and
      • Outdoor activities and special events;
    • Comprehensive Accessibility:
      • Transportation demand management plan;
      • Multi-modal transportation plan; and
      • Parking, loading study;
    • Conservation:
      • Historic considerations;
      • Sustainability considerations; and
      • Environmental impacts;
    • Landscape/Open Space;
    • Streetscape treatment, including signage/wayfinding;
    • An explanation of all programs on campus including educational, certificate, adult learning, international, career training, religious, night classes, civic, and non-profit uses;
    • The number of students enrolled in a District of Columbia public or public charter school that also take classes on the campus shall be included in the application but shall not be counted against any approved enrollment cap (if applicable); and
    • The names and mailing addresses of the owners of all property within 200 feet, in all directions from all boundaries of the property involved in the application, and two (2) sets of self-stick labels of the names and mailing addresses of the owners of the properties. (Note: This information is most readily available from the D.C. Department of Tax and Revenue, Tax Assessors by using the following link: . Select “View More Options” under the Real Property section, then “Request a Radius Map” under the Real Property Quick Links section.)

Please note, after you submit this electronic filing, you will be required to submit two paper copies of the application with supporting documents, unless you are including plans, in which case eight copies are required. Also, at the time of filing you will be required to serve a copy of your application on the Office of Planning and the affected Advisory Neighborhood Commission 

The application will not be accepted for processing unless accompanied by a certificate of service demonstrating that a copy of the application and all accompanying documents have been served upon: 

  1. The Office of Planning; and
  2. The affected ANC. 

Note: All applications are referred for review and recommendation to the Office of Planning (OP) and the Advisory Neighborhood Commission (ANC) within which the affected property is located. Their reports are given “great weight” in the Zoning Commission decision-making process. Applicants are strongly encouraged to contact these agencies to discuss the merits of their application/petition. OP can be reached at (202) 442-7600.
